The Mark Schacht Diabetes Log© is FREE to individuals with diabetes, or to their immediate family members, provided that prior to downloading you send me an email acknowledging that you will use the form ONLY as a personal health tool in monitoring your own, or your family members', diabetes, and will not sell, give, or distribute it to anyone else.

STEP 1. CLICK HERE TO E-MAIL MARK SCHACHT (by clicking on E-MAIL)and simply say you're downloading the form for your personal or familial use. (Your email address will not be provided to anyone, nor will I use it to email you unless you indicate you wish to receive updates on changes to the form.) Then return to this form and follow the next two steps...
STEP 2A. TO ACCESS THIS .PDF FILE, your computer's Internet Browser MAY have Adobe Acrobat Reader installed as a "plug-in." While many newer computers' pre-installed Internet Browers come with "Adobe Acrobat Reader" already installed, if it is not already installed, you will have to go to Adobe's site and download and install it. (Note: you will get error messages if you try to download the file and your browser doesn't have the Acrobat Reader "plug-in" installed.) If Reader is not installed go to 2.b.; if so, go to 3.
2B. You can download the (free) Acrobat Reader "plug-in" simply by clicking here HREF> and following instructions. (Once it is installed, you won't have to do this ever again.)
STEP 3. AFTER YOU'VE INSTALLED the Acrobat software, click here to download HREF> the Mark Schacht Diabetes Log to your computer. Once it has finished downloading, an Adobe Acrobat Reader window should open displaying the form. Click the Print icon (but only the one on the tool bar at the top of, but within, the Acrobat Reader window, not the one in Window's or in your Brower's pulldown menu).
STEP 4. SAVE THE FILE FOR FUTURE USE. Use your file manager to create a new folder called "Diabetes Log" and copy this .pdf file to that folder. To open the file again, to print more copies, just simply double-click on it.
